The Welland McMaster Family Health Team is looking to hire two (2) Social Workers with a Master's Degree in Social Work and a current membership with the Ontario College of Social Workers and Social Service Workers to join their multidisciplinary team: so here's your chance! Social Workers will have the opportunity put to use their program development, implementation, monitoring and evaluation skills. The opportunity, in a nutshell: Social Workers at the Welland McMaster Family Health Team assist in the primary care functions and participate in the development, implementation, monitoring and evaluation of programs and services to support the patients of 10 family physicians. There will also be occasions to provide programs to the community. The Welland McMaster FHT is a teaching FHT so Social Workers will have the opportunity to work with various students and residents. The Social Workers will practice within the scope and according to the standards of practice as outlined in the College of Social Workers and Social Services Workers. The required qualifications & education needed for this role: Master's Degree in Social Work from a recognized University. Member in good standing with the Ontario College of Social Workers and Social Services Workers; and Proof of professional liability insurance. Three to five years' counseling/therapy experience: preferable in a community setting. This is to include proven experience with current, evidenced-based therapeutic intervention methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction. Current Ontario Driver's License. Proficiencies in assessment, psychotherapy and psychological counseling techniques for working with individuals, couples. Families/communities. Communicates effectively with health care team members to create a cohesive team and provide seamless services to patients and community groups. Communicates effectively with all clients, families, peers, other health care professionals and community partners. Participates in interdisciplinary meetings. Proven leadership, organization, research, evaluation, time management, communication and interpersonal skills; Additional assets: Proficiency in the use of computer hardware and software, particularly in Microsoft Office Suite: Word, Excel, and Outlook.
